Hospice Care is the specialized comfort care provided to the terminally ill. It is a holistic, patient-centered approach, which includes physical and spiritual support for both the patient and family. It focuses on quality of life so that every day is lived to its fullest.
Care is provided for a wide variety of disease processes in many different community settings such as homes, skilled care nursing facilities, and hospitals. The Hospice Team is comprised of caring and compassionate nurses, social workers, a spiritual counselor, home health aides, dedicated volunteers, and includes the assistance of other Home Health services when necessary.
Bereavement support is provided for up to 13 months after a loved one’s passing. Surviving loved ones are supported through services including short-term grief counseling, telephone counseling and assistance with memorial planning.
